Medical vs. Surgical management with volar plate in elderly patients with distal extra articular radius fracture

Distal radius fractures are one of the most frequent injuries found it emergency service. Conservative treatment was very frequent until few years ago. However in past decades we have seen a progressive tendency to surgical treatment of this type of fractures, because of multiple complications that came with conservative management.

Objective: to compare the evolution of 50 years or older patients with distal extra articulate fractures of distal radius who were treated with conservative vs. volar plate, and attended in external consultation of orthopedics and trauma department in Hospital Escuela during the period between June 2009 and June 2010.

Methods: descriptive, longitudinal study, with a 27 patients sample with diagnosis of a distal extra articulate radius fracture who were treated with a conservative method because of evolution time, vs. patients treated with volar plate, both groups were evaluated in two and three month after the first treatment. We analyze radiological changes, function, pain and consequences related to both methods.

Results: The majority didn't present changes in angulations and radius height. In both treatments the most common consequence was the articular range motion. Patients treated with volar plate could do their daily activities with low grade difficulty, however patients treated with conservative management 12 weeks after the fracture, had moderate difficulty to do their activities

Conclusions: patients treated by surgery presented less alteration in radiological evaluation and have better function 8 weeks after surgery because of rigid stabilization and early movement

Surgical Management and Complications of esophageal injuries: Study of Cases in Hospital Escuela, Honduras

OBJECTIVE: Establish the prevalence and surgical treatment of esophageal injuries and the morbidity and mortality associated.

PATIENTS AND METHODS. Descriptive study involving patients admitted to the surgery service of Hospital Escuela from January 1st, 2008 through june 30th, 2010 with diagnosis of esophageal injuries. We applied a data collection instrument for clinical findings and patient management. The results are presented as frequencies and percentages of study variables using the data processing system EPI.INFO 3.5.1

RESULTS. A descriptive clinical review was made of all patients with perforation of tha esophagus. There were 11 patients the most of them males an average age of 29 years. The principal cause of injury was gun shut 64 % of cases follow by foreign body ( 27%). Cervical esophagus was the most affected area 64% follow by the intrathoracic esophagus. Grade III injuries were the most frequent and it was manage with suture of the perforation. Esophageal exclusion was performed in the 36.5% of the cases this due to the severity of tissue damage found. Image studies performed were cervical X ray, thoracic projections and esophagography. The most serious complication of this injuries was the mediatinitis with a high mortality in 4 of the 11 cases.

CONCLUSION. Esophageal injuries are rare conditions with a high mortality that makes important to diagnose and treat this injuries as soon as they are found.
